Learning Objectives:

At the conclusion of this session, participants will be able to:

understand common HFrEF phenotypes;
know how to individualize patient care based on phenotypes; and
understand how to apply GDMT based on phenotypes.

Comprehensive Guidelines, Guideline Updates and Position Statements by the Canadian Cardiovascular Society (CCS) and Canadian Heart Failure Society (CHFS) have been published over the last decade to support optimal diagnosis and management of patients with heart failure (HF), generally. It is becoming increasingly evident that the clinical management of HF and the trajectory of disease progression also depends on specific patient characteristics, traits and clinical presentations, defined as HF phenotypes.

In this session, led by Canadian HF experts, we will focus attention on the important clinical phenotypes of HF and how the health care team can recognize them and provide phenotypic-based care for patients to optimize outcomes.

Specifically, this session will focus on the following HF phenotypes: 1) The wet and hypertensive HF phenotype, 2) The de-novo HF phenotype; 3) the HF with cardiorenal syndrome phenotype 3) The worsening HF phenotype.

Faculty will provide real world examples of these clinical HF phenotypes, offer practical advice on how to identify these patient phenotypes in practice and how to deliver personalized care using the CCS HF Guidelines as a framework.
