At the conclusion of this session, participants will be able to:

discuss the Burden of Heart Failure particularly in the readmitted patient;
review CCS Guidelines on management of Heart Failure and Foundational Therapies;
describe the limitations of some therapies and possible solutions; and,
discuss possible practical issues around identification of appropriate patients and subsequent ways to optimize Guidelines Base Medical Therapy in those patients.

This session will explore the burden of Heart Failure Readmissions while highlighting the importance of adhering to the CCS Guidelines.
