The foundation for accurate clinical documentation and prudent patient care is the medical history. Many clinicians do not know how to obtain a truthful and complete medical history from their patients. They also do not spend the adequate time or use the appropriate resources required to achieve the goal of understanding the medical condition of the patient prior to treatment. This program will dramatically demonstrate the serious consequences of incomplete and ignored medical histories for patient and doctor.
