Introduction: The oncological safety and technical feasibility of robotic proximal gastrectomy (RPG) with double tract reconstruction (DTR) is largely unknown. Compared with total gastrectomy, theoretical benefits include gastric preservation, decreased reflux and improved nutritional status.

Methods: We report a 39 year old male with locally advanced Siewert II gastroesophageal junction (GEJ) invasive adenocarcinoma. After excellent response to neoadjuvant chemotherapy, he underwent RPG with DTR. Final pathology showed partial response with adequate margins.

Results: The patient’s p</span>ost-operative recovery was favorable. Upper GI series demonstrated patent anastomoses without leak or obstruction. He reported stable weight, improving appetite and adequate food intake at outpatient follow up.

Conclusions:

In selected patients, RPG with DTR is technically feasible. Further studies are needed to evaluate long-term oncological outcomes and quality of life for this approach.
