A single arm prospective QA assessment was carried out in lower extremity wounds greater than 4 sq cm. BPMAg (Microlyte Ag®) was applied for 4 weeks. Some patients had the dressing applied as frequently as 3 times a week, at home. While another group had the material applied weekly under compression. The secondary dressing for these highly exuding wounds was a highly absorbent dressing (Optilock ®).
Results:
: Thirty patients were enrolled, there was a 72% mean wound area reduction at the end of 4 weeks. Six of 30 patient (20 %) did not respond significantly. The patients had a reduction in pain and drainage. No patient required the highly absorbent dressing and the end of therapy. All (100%) of the wound had a significant reduction in the amount of drainage. The only wounds that did not reduce in size were the wounds that had a significant dermal component. The median number of applications was 2 per week.
Discussion:
The BPMAg was very effective in reducing the amount of drainage in all (30/30) wounds. The wound bed prep and depth probably effected the closure rate in 20% of wounds. The 20% which did not show area reduction all involved the dermis. The ease of use, and the ability of home care provider (including self application, or family member application) application made this dressing very attractive. The product showed highest efficacy in partial thickness wounds, such as exuding dermatitis and skin tears.
