Introduction: Hidradenitis Suppurativa is a chronic suprative skin condition involving the apocrine sweat glands which is difficult to treat. Many options exist with variable success.
Methods: The proposed surgical treatment is minimally invasive requiring only local anesthesia an can be performed in the office/clinic out-patient setting. The suprative lesions are injected with 2% lidocaine without epinephrine. A 4mm punch biopsy instrument is then used to completely excise the infected gland. Multiple excisions can be performed at the same time. No antibiotics prescribed. The excised sites are then cleaned with wound cleanser and covered with a medicated foam dressing.
Results: No infection or recurrence was seen at the excision sites. Preserving skin bridges between excision sites allows for faster wound healing. Minimal post-procedural discomfort or pain has been noted.
Discussion: A problematic medical condition can be dealt with in an out-patient setting with good results. The procedure can be repeated on additional lesions. This procedure is indicated for Hurley type I and II lesions.
