For many of us working in acute care or wound centers we frequently send patients into other sites of care with orders and expectations that may be unrealistic or at best difficult in these other sites. This case-based session will look at the real challenges encountered in the home and long-term care settings and how we can enhance communication and improve partnerships with care providers of these challenging patients.

Learning Objectives:

Identify unique challenges seen in the post-acute care wound patient.
Generate a variety of possible solutions which improve quality outcomes, patient satisfaction, and adherence in the post-acute setting.
