Innovation Theater Breakfast (non-CME): Diabetic Wound Healing of a Chopart’s Amputation Site Using Amnion Membrane Wrap and Dermistat Autogenous Graft - From Necrotizing Fasciitis to Full Ambulation
Neglected diabetic foot ulcerations are commonly associated with severe soft tissue infection such as necrotizing fasciitis and oftentimes concomitant with osteomyelitis which eventually leads to partial or complete limb loss. The difficult aspect to treating extensive tissue and bone infection is determining the level of appropriate amputation in a timely manner, which then followed by good wound care. Each patient’s physical functions, daily activities, vascular status, associated comorbidities and overall mental expectations differ greatly, thereby complicating the goal of limb salvage. A Chopart’s amputation preserves limb length and ankle function. However, with a Chopart’s amputation there are often complications associated with wound healing and high risk of recurrent ulceration. Using Amnion Membrane Wrap and Dermistat autogenous tissue graft, our case study showcases the level of accelerated healing and return to function after a radically large soft tissue deficit with exposed bone. In this presentation we will explore how our chronic diabetic and neuropathic patient with a noted complex medical history and neglected wounds, who initially presented with necrotizing fasciitis, was able to benefit greatly from a Chopart’s amputation followed by serial application of amniotic tissue biological graft dressings, that have since reduced the size of the wound to the point that patient has return to full amputation with a custom made brace.
