Most people who struggle with severe mental illness (SMI) have high rates of chronic medical illness and are woefully unengaged in an ongoing stable primary care relationship like those offered by community-based providers. We will describe successful case studies from Missouri and Tennessee. What has worked? What are states and managed care organizations (MCOs) doing well for these people? What will success look like in 10 years, if and when these problems are solved?
