Create continuous coverage and services for justice-involved people as they exit the justice system by leverage local, state and federal efforts. Major policy changes seek to remove structural barriers and ensure that health care coverage and services can begin while an individual is incarcerated. We will discuss the historic/structural barriers to improving care for justice-involved people, local efforts to improve care and changes ahead. This session is ideal for clinicians, state and local health authorities, and C-suite who are grappling with how to deliver services to justice-involved populations.

Learning Objectives:

Describe efforts between COCHS, West Hawaii Community Health Center, Bay Clinic and Kumukahi Health and Wellness to improve care and coordination for justice-involved individuals.
Analyze the federal and state changes in Medicaid service delivery while incarcerated and the effects that it will have on providers and health systems.
Identify opportunities locally and statewide to prepare for implementation of the policy changes to the inmate exclusion that will improve care coordination and health care delivery while incarcerated and upon release.
