Jennifer Genovese is the director of health care integration for Innovative Care Partners, a Massachusetts-based community partners program and has extensive experience with care coordination and managing programs that focus on providing integrated services to at-risk Medicaid individuals. The focus of these community-based services includes utilizing data-driven interventions in order to help drive our focus and lower acute admissions and emergency room visits, improve connection to medical and behavioral health providers and address the social determinants of health present in Innovative Care Partners’ program population. Helping their participants navigate systems, understand treatment goals, and engage in the complex system of care with aid of care coordinators within the Medicaid population is an integral piece to lowering health care costs and improving health outcomes.
Prior to joining Innovative Care Partners, Genovese was involved in the development and growth of the Integrated Care Management Program at the Massachusetts Behavioral Health Partnership, a Medicaid Managed Care Organization in Massachusetts. The Integrated Care Management Program (ICMP) is an enhanced care management program offered to Primary Care Clinician Plan members with complex medical, mental health and/or substance use disorders. Clinical staff provided integrated medical and behavioral health care management, which included direct face-to-face care management visits with members. Similar to the Community Partner Program, predictive modeling tools analyzed behavioral health, medical, and pharmacy claims data to assist in the identification of members with the need for integrated physical and behavioral health services and care coordination.
