A year ago, IMSNY won the NatCon Shark Tank with a presentation on how a well-planned data analytics strategy serves as the foundation for an agency or network seeking to systematically improve how care is delivered. We will share insights from our data journey, with a special focus on behavioral and social determinants of health. We don’t promise an “easy” button, but we do commit to an honest presentation of challenges and some pathways to overcoming them.
Learning Objectives:
How bringing together and (re)viewing the "right" data supports a health network in achieving financial value and improving health outcomes.
Existing tools, programs, access, processes and outcomes to leverage existing data for more comprehensive measurement analysis, risk stratification and intervention implementation.
Common pitfalls in data analysis, especially as it relates to behavioral health and social determinants of health.
Types of data available and how a network might organize them to identify opportunities to improve care.
