Session Description: Achieva will share how it is using a community of practice to chart the course for transformation of its organization and the service delivery system in Pennsylvania. Through participation in a community of practice, Achieva has embraced a more person-centered culture that promotes access, equity, and inclusion across all services. It has also incorporated the use of Charting the LifeCourse tools to engage the people its support in planning for their best possible life.
