(P 219) PROGNOSTIC VALUE OF HEMATOLOGICAL MARKERS IN LEIOMYOSARCOMA

Objective: The main objective is to determine the prognostic value of hematological biomarkers based on their NLR (neutrophils/lymphocytes), LMR (lymphocytes/monocytes), PLR (platelets/lymphocytes) and SIRI (systemic inflammatory response index) ratios at diagnosis, in patients with confirmed leiomyosarcoma prior to surgical intervention.
Secondaries:
- Determine the recurrence of the disease in these patients based on the values of their hematological markers.

Methods: Uni-institutional retrospective study in patients diagnosed with leiomyosarcoma in which hematological biomarkers were quantified prior to surgical treatment obtained from routine blood tests requested preoperatively.
ROC curve analysis was performed to determine the optimal cut-off points in the different ratios, taking the value of the union of maximum sensitivity and specificity (Youden index).

Results: Following the ROC curves, we found values for the area under the curve (AUC) for NLR of 0.804 (confidence interval [CI] at 95%: 0.68-0.928, p< 0.01), SIRI of 0.765 (confidence interval [ CI] at 95%: 0.632-0.898, p=0.01), PLR of 0.637 (confidence interval [CI] at 95%: 0.482-0.791, p=0.095) and LMR of 0.282 (confidence interval [CI] at 95 %: 0.136-0.428, p=0.008), so we consider with these data, the NLR and the SIRI, as good ratios for the diagnosis of non-survival, since they have an AUC >0.75. The Youden index calculated on these values:
NLR: 2.97. S= 0.813; 1-E=0.333
SIRI: 600882.35. S= 0.875; 1-E=0.333
Based on the cut-off points identified, we found a higher mean survival measured in months for the group with NLR < 2.97, compared to the group with NLR >2.97 (p= 0.003). Similarly, a lower survival was observed in those patients with SIRI >600882.35 compared to those with SIRI < 600882.35 (p=0.004) (Graph 2).
Survival at 1, 3 and 5 years for both groups in both biomarkers was statistically higher in the groups with values below the cut-off point (Table 1).

Secondly, the ROC curves of these same indices were made in relation to the recurrence of the disease, with the AUC < 0.75 in all cases and with no significance, so that in our study these ratios have not been effective for predict recurrence (Graph 3).

Conclusion: The findings of our study suggest that elevated NLR and SIRI, determined before the start of treatment, can be considered poor prognostic factors in patients diagnosed with leiomyosarcoma. None of the hematological markers studied has been able to relate to a prognostic factor for recurrence in this type of tumor.
