Improving Catatonia Recognition Using Video Vignettes

Abstract: Catatonia may lead to significant morbidity or mortality. Accurate recognition in both psychiatric and medical settings is imperative so that underlying medical and neurological illness can be addressed, and that catatonia can be treated with lorazepam, other medications, or ECT. However, despite availability of reliable standardized instruments such as the Bush-Francis Catatonia Rating Scale (BFCRS) (1), catatonia remains widely under-recognized in both psychiatric and medical settings (2). Given that catatonia is a psychomotor syndrome, education to improve identification of catatonia should prioritize audiovisual depictions of its clinical features. Therefore, we will dedicate most of this interactive workshop to video-based examples of catatonia to help participants hone their skill in recognizing catatonia and differentiate among its unique features. In this session, we will encourage liberal audience participation through questions and real-time dialogue.

Dr. Oldham [presenter, 15 min] will review results from a recent online educational module that identified critical gaps in understanding the features of catatonia (including among C-L psychiatrists) (3) and the effectiveness of an online video-based module in improving recognition of its features (4). He will also orient attendees to the BFCRS Training Manual and Coding Guide and review a set of newly developed model-patient videos on identifying and scoring the severity of all 23 individual BFCRS items (5).

Dr. Francis [moderator], co-developer of the BFCRS, will moderate a 75-minute interactive discussion, showing and reviewing a series of video vignettes of actual medical and psychiatric patients with catatonia before, during, and after treatment.

Dr. Mormando [discussant] will provide expert commentary on common and unusual features of catatonia as displayed in the video segments, and all three session presenters will review key findings by panel discussion.
