Purpose: : CHH’s objective was to reduce the CLABSI event rate to 0% during the COVID pandemic by: - Stopping migration of bacteria into the catheter sites when patients was turned - Reduce frequency of unscheduled dressing changes due to loose dressings
Methods: : The antimicrobial tissue adhesive trial began March, 2021 in our 20 bed, Medical ICU. The antimicrobial tissue adhesive representative educated the staff on proper application to the catheter site and dressing using sterile technique. Several tissue adhesive applications were supplied to the MICU from March to the beginning of November. The nurses used the antimicrobial tissue adhesive and reported their evaluations to the floor manager. These evaluations were logged. The patient infection rate was monitored and recorded to NHSN by the Infection Preventionist.
Results: : The 6 month trial was from the beginning of March, 2021 to the beginning of November, 2021. MICU NHSN SIR rate of 1.232 was reduced to 0.609. There were two CLABSI events noted with one at the beginning of the trial and one at the end of the trial when the product was unavailable.
Limitations:: Unfortunately, during the trial, the supply personnel were unable to track the dressing supply due to a change in the supply database. The MICU clinical coordinator did state, during the trial, she observed a substantial reduction in unscheduled dressing changes with the usage of an antimicrobial tissue adhesive.
Conclusions: : The tissue adhesive did reduce the CLABSI event rate for the MICU. Additionally, the MICU nurses were highly satisfied with the product. 79% of 14 nurses who turned in an evaluation, gave the product a 5 out of 5 for ease of application, effectiveness, and overall acceptability. All recommended the product. Several expressed how the dressing stayed intact even with bodily fluids on/around the dressing. Based on the results, the tissue adhesive was approved for hospital-wide use. As seen in Figure 3, the MICU has been CLABSI free since December 2021 with the usage of the tissue adhesive.
