050IC - Surgical Considerations and Coordinated Medical Management of Difficult Retroperitoneal Tumors

We will detail techniques to obtain total vascular control in the retroperitoneum to minimize blood loss and efficiently remove difficult retroperitoneal masses. Additionally, we will display how to control difficult bleeding from the great vessels . Options after failure of induction chemotherapy and failure of standard salvage chemotherapy present difficult decisions between surgery or high-dose chemotherapy. The approach to these decisions and important clinical considerations will be discussed using guideline-based considerations and situations that go beyond the guidelines. In addition, this course will describe the approach to the retroperitoneum for patients with high-grade retroperitoneal sarcomas including a discussion of neoadjuvant and adjuvant therapies. Finally, a discussion of the urologist’s approach to incidentally discovered retroperitoneal masses on imaging will occur, including what clinical features should be considered.
