Introduction: Indocyanine green dye (ICG) lymphangiography has significant impact to the Robotic Assisted Radical Prostatectomy (RARP). We can use this technology to help identification of lymph nodes with lymphadenectomy. We report the result of lymph node metastasis correlation with ICG lymphangiography assistant in prostate cancer patients.
Methods: Cystoscope guided intraprostatic injection was performed successively in 42 localized and locally advanced prostate cancer patients in this study. The mean age was 69.1±5.66 years old. The patients number of high, moderate and low risk were 20,14,8. Total 638 lymph nodes were identified. 364 lymph nodes were ICG positive and 274 lymph nodes were ICG negative. There were 56 metastatic lymph nodes found in 7 patients’ lymph nodes (all high risk group patients) . The sensitivity of ICG positive in metastatic lymph node is 89% (50/56). The positive predictive value of ICG positive lymph node is 14% (50/364) and negative predictive value of ICG negative lymph node is 98%(268/274)

Conclusions: Cystoscope guided intraprostatic ICG injection with fluorescence Lymphangiography can help identify pelvic lymph nodes in Robotic Assisted Radical Prostatectomy. ICG lymphangiography can assist identified metastatic lymph nodes in pelvic lymph node dissection during Robotic Assisted Radical Prostatectomy.
