Objectives: This study aimed to evaluate associations of whole-diet maternal dietary inflammatory index (DII) with pregnancy outcomes.

Methods: The analysis included 7 316 mother- neonate pairs in Shaanxi Province of Northwest China, a cross-sectional study. Maternal diets during pregnancy were assessed based on food-frequency questionnaire and DII calculated by standardizing the dietary intakes of each participant to representative world database means, which were multiplied by food parameter-specific inflammatory effect score and summed. Pregnancy outcomes, involving cesarean delivery, pregnancy-induced hypertension (PIH), birth weight and lower birth weight (LBW) were obtained by reviewing medical records and infant birth certificates. The strength of associations between DII and pregnancy outcomes were estimated using generalized linear models.

Results: Mothers had a mean ± SD age of 27.4±4.6 years, DII was -0.01±2.65 units ranging from -6.2 to 5.8. Higher DII scores, indicating more proinflammatory dietary potential, were associated with decreased birth weight (adjusted b=-5.36 g; 95% CI: -9.34, -1.38 g) and higher likelihood of delivering a LBW infant (adjusted OR=1.06; 95% CI: 1.01, 1.12), whereas the DII was not linked to the rate of cesarean delivery (OR=1.00; 95% CI: 0.98, 1.02) and development of PIH (OR=0.99; 95% CI: 0.92, 1.06) after adjustment, respectively.

Conclusions: Maternal diet with higher inflammatory potential may be associated with lower offspring birth weight.
