Primary progressive aphasia (PPA) is a progressive neurological syndrome with three variants: logopenic, nonfluent/agrammatic, and semantic. We hypothesized PPA variant could be confirmed by differences in volume between an individual’s right and left hemisphere in specific regions of interest. Left:right asymmetry in inferior temporal gyrus pole (ITGp), temporal gyrus pole (MTGp), and inferior temporal gyrus (ITG), distinguished svPPA from other variants. Left:right asymmetry in inferior frontal gyrus (IFG) or IFG + insula (IFG-ins) identified individuals with nfvPPA. Based on a multinomial regression, left-right differences in angular gyrus, supramarginal gyrus, and superior temporal gyrus contribute to distinguishing PPA variants. This study provides preliminary evidence for quantifying left:right atrophy in regions associated with PPA variants to optimize classification and medical management.
