It can be challenging to capture the salient pieces of information that assure a safe transition of care for the patient. It’s even harder to make certain all clinical and compliance expectations are present for the assorted value-based payment models. During this session we discuss the core components of essential clinical documentation. Giving evidence of true interprofessional collaboration has become the hallmark of quality documentation. Clinicians will use this information to ensure that their multi-disciplinary interprofesional team understands their value, that care is coordinated across clinicians and settings, and that their patients are getting the highest quality care.
