Transgender Medicine and HIV – Clinical Pharmacology Considerations

Session Description

Transgender people are disproportionately affected by HIV, with about 20% of transgender women living with HIV and many transgender persons potentially benefiting from effective HIV prevention. Globally, transgender gender-nonbinary populations are becoming more visible, and gender-affirming therapies are becoming increasingly accessible and used. Several barriers and disparities in medical management of HIV exist for transgender care, including underrepresentation in clinical research, stigma and discrimination that impede effective use of HIV treatment and prevention, and concerns among transgender persons about that potential for adverse drug-drug interactions between HIV treatment or prevention medications and hormonal medications used for gender-affirming care. As new medications ae developed for HIV treatment and prevention, understanding whether those medications interact with gender-affirming therapies will be essential to effectively engage transgender persons in care. This session will address transgender care as it relates to HIV treatment and prevention care, with specific attention to unique pharmacologic considerations for transgender individuals undergoing gender-affirming medical care and potential influence on the safety and effectiveness of antiretroviral medications used in HIV treatment and prevention.

Learning Objectives

1. Describe basic terminology and medical interventions in transgender medicine and identify the effect of gender-affirming medical interventions on antiretroviral disposition among transgender adults

2. Describe the potential drug interactions between hormonal therapy and antiretrovirals in transgender adults and discuss inclusion of transgender populations in clinical research.
