Clean Slate is an outpatient Addiction Medicine Practice. We provide comprehensive care and offer medication assisted treatment, individual/group counseling, recovery coach services, and care coordination services. We take care of special patient populations including pregnant patients and treat co-occurring disorders such as depression.
