Full Description: According to the 2018/19 National Survey of Children’s Health, only 23% of youth ages 12-17 with special health care needs and 17% of youth 12-17 without special health care needs received services necessary to make transitions to adult health care. In 2021, 33 state Title V programs prioritized national performance measure (NPM) #12 to address these low rates of health care transition (HCT) preparation among adolescents through evidence-informed strategies. This workshop will highlight Got Transition’s new findings from its 4th annual survey of HCT implementation in care coordination programs of these state Title V agencies. The session will also offer strategies and suggestions to state Title V programs for future HCT efforts and improvements. This qualitative survey is conducted annually to assess state Title V care coordination HCT trends.
Specifically, this workshop will cover the 2021 findings compared to results from previous years (2017, 2018, and 2019) as they relate to HCT implementation in care coordination programs. The workshop will also include a discussion of identified key themes about the impact of COVID-19 on HCT implementation and programming. This survey, completed with 100% response rate, is unique in that for the first time, each state was asked about the impact of the coronavirus pandemic on their HCT efforts from June 2020-July 2021. All states reported that they were affected in some way, setting the stage for reflection of the past in order to understand and shape future HCT efforts.
State Title V agencies and their care coordination programs and partners play a critical role in the implementation and spread of comprehensive HCT supports for youth with special health care needs and their parents/caregivers. After this workshop, state Title V representatives will have a deeper understanding of their current HCT performance and how to move forward with next steps, in light of COVID-19. Got Transition will also provide customized electronic profiles of each state’s HCT performance since 2017 to attendees. This session relates to the conference theme as it is intended to reflect on and compare findings from past years performance of this survey and use that knowledge to structure future strategies around HCT implementation.
