G7 - Shaping the Future of Florida’s Patient Family-Professional Partnerships Through a Statewide, Interdisciplinary Quality Improvement Initiative

Full Description: The intent of this workshop is to share key learnings and accomplishments from the Florida Title V Children and Youth with Special Health Care Needs (CYSHCN) Program’s Learning Action Network (LAN). This includes a unique and essential resource developed by the presenters, “A Roadmap to Inviting, Engaging, and Including Patient/Family Partners in Quality Improvement and Other Health Related Initiatives.”
Florida’s Department of Health Office of Children’s Medical and Specialty Services (CMS), that includes the CYSHCN Program, shifted focus to effecting population health impact a few years ago. One such shift is the cutting-edge project, the FL CMS LAN. This initiative, in partnership with the National Institute for Children’s Health Quality (NICHQ), leads a statewide interprofessional Quality Improvement (QI) LAN that works with seven Statewide Networks for Access and Quality (SNAQ). The vision of the FL CMS LAN is every child with medically complex conditions (e.g., HIV, kidney disease, diabetes) receives high-quality, evidence-based, family-centered medical care, regardless of health insurance or where they live.
From the outset we embraced the views of the MCH Leadership Competency on Family-Professional Partnerships, valuing family partnership as an essential element of health equity. Family leaders were included in the development and start-up phase of the LAN program and were involved in all planning meetings and LAN events from start to present. Training, technical assistance, and resources have been infused throughout LAN activities, with the understanding that each of the 31 clinical teams within the seven different SNAQs include at least one family partner in their local clinic’s QI project.
The interactive panel presentation will engage audience participation with polls, group discussions, and idea sharing throughout the session. Accomplishments, lessons learned, and key takeaways will be shared. Participants will gain a broad overview of this effective QI program, including the growth of skill attainment and application of QI processes by key populations impacted by this venture – providers and clinical staff, family partners, and CMS Title V staff. The workshop showcases meaningful growth of family engagement and partnership over time and how to support this process.
Relevant to family-professional partnership will be coverage on steps taken to include family leaders on the co-design of increasing family partnerships, including creating a brand-new “Roadmap” resource. Many CMS Title V staff are involved as liaisons with the SNAQ teams for this project. Thus, further relevance to the AMCHP audience is workforce development on QI, family engagement practices, and team facilitation skills.
From program officers to quality improvement staff, and from providers to families with lived experience, this session moves from past reflections which informed our “why” to discuss current accomplishments and share future plans for the FL CMS LAN.
