Recognizing Errors to Improve Patient Outcomes

Medical errors can save lives as they elucidate where our procedures and systems fail to protect our patients. We will cover systems-based approach to error and root cause analysis – determining why an error really happened, with all of the contributing factors far beyond the point of care to the patient. Case examples will be discussed to demonstrate key principles.
