An interdisciplinary battery of standardized outcome measures has been implemented in routine care in inpatient stroke rehabilitation using knowledge translation strategies. Based on identified clinical practice gaps, an activity and intensity-based interdisciplinary treatment program called the "Leveraging Intensity for Treatment" program has been tailored to the unit's needs and systematically implemented. The symposium will present an overview of knowledge translation and strategies that can be used to promote the implementation and sustainability of evidence-based practice in routine clinical care. A unique insight into the activity level of adults with stroke in inpatient rehabilitation in Norway will be provided and experiences from implementing standardized outcomes and new interventions in both occupational therapy and nursing. Finally, the symposium will present an analysis of the sustainability of practice two years after high-intensity gait training was implemented in routine care.
