The purpose of this symposium is to describe the development and implementation of a process to improve communication amongst health care providers when patients with a cancer diagnosis transition from acute care hospital to acute inpatient rehabilitation. Our symposium will provide 1) support for the creation of a novel cancer continuity of care form, 2) description of the form contents, 3) strategies to implement a continuity of care form into practice, 4) application of form contents in the context of an established cancer clinical pathway, 5) explore barriers/facilitators to successful implementation, and 6) provide future steps to investigate form utility. We will then explore case examples from an interdisciplinary perspective that demonstrate the potential impact of a cancer communication tool on plan of care development in the inpatient rehabilitation setting.

Learning Objectives:

Describe the challenges faced by post-acute care rehabilitation providers when initiating plans of care with patients with cancer.
Describe the development of a cancer-specific electronic medical record Continuity of Care process.
Identify key elements in the Continuity of Care process which facilitate information transmission to post-acute care providers working with patients with cancer.
Describe the clinical application of the Cancer Continuity of Care process within an inpatient acute rehabilitation setting.
