For those who survive the COVID-19 virus following medical intervention, symptoms range from mild to severe (Hu et al., 2021), and persistence of symptoms has been an increasingly recognized problem (Mendelson et al., 2020). Two years into the pandemic, the literature produced on COVID-19 has highlighted the diverse symptoms and acute and prolonged dysfunction that may ensue from the COVID-19 virus, which most commonly involves physical, cognitive, and psychiatric symptoms. Rehabilitation team models have been proposed for to address post-acute sequelae of COVID-19 infection. This symposium provides an in-depth understanding about an interdisciplinary model of care during which operational clinic development and workflow and clinical characteristics of the participants are discussed.

Learning Objectives:

Provide an understanding about the operational development and workflow within a post-COVID clinic interdisciplinary model of care addressing persistent COVID-related symptoms.
Examine clinical profiles through clinician rated and patient reported outcomes.
Review a plan of care and tailored treatment paradigms for physical, cognitive, and psychological symptoms identified within the interdisciplinary assessment.
