A. Personal Statement
I am currently the VA Puget Sound HCS Chief of the Anesthesia and Pain Medicine Service Line and the Director of the VISN 20 Pain Medicine and Functional Restoration Center. I am the Chief Anesthesiology Consultant for VISN 20. I serve as a Clinical Associate Professor of Anesthesiology and Pain Medicine at the University of Washington School of Medicine. Prior to my current position I was the director of a private practice, Northern California Pain Care, for seven years in the San Francisco Bay Area. During that time, I also served as a Clinical Assistant Professor of Anesthesiology and Pain Medicine (part-time faculty) at the Stanford University School of Medicine. I also serve as the Chair of the VISN 20 Opioid Safety and Pain Management Committee, the Chair of the VAPSHCS Opioid Safety Review Board, Director of the VISN 20 Pain Mini-Residency for Primary Care and serve on the VHA National Pain Committee. My clinical background in Pain Medicine and in the VA will help me serve as a member of the Stakeholder Advisory Group and direct how the study findings can best be incorporated into VA practice.

1. Development, outcome measures and evidence-based practice for interdisciplinary pain care and functional restoration. I have been actively involved over the past decade in the development of curricula and program deployment as well as outcomes analysis of functional restoration programs for the interdisciplinary treatment of chronic pain.

2. Understanding the efficacy and predictive value of interventional pain management procedures for the treatment of lumbar and sacral pain. I have participated as a local PI or co-investigator on multicenter and individual center trials, looking at the effectiveness and predictive value of lumbar medial branch blocks and lateral branch nerve blocks for sacroiliitis and the effectiveness of radiofrequency denervation for lumbar facet arthropathy. This includes a pilot randomized trial of lumbar radiofrequency denervation, led by Dr. Suri, not yet reflected in my publication record.
