A. Personal Statement
From 8/2007 through 5/2014, I was Director of the Pain Clinic at the Central Texas Veteran Health Care System. I subsequently relocated to Seattle where I was Administrative Director of the Spine and Musculoskeletal Clinics, Puget Sound Veterans Health Care System from 6/14/2022 through 4/30/2022. I continue to perform video telehealth consult services for the Puget Sound Veterans HCS, Rehabilitative Care Services. During my tenure at both locations I was actively involved in the performance of spinal interventional procedures and also trained Residents from the University of Washington Department of Rehabilitation and Fellows from the University of Washington Department of Pain Medicine in spine and musculoskeletal medicine, pain medicine, and spinal and peripheral interventional procedures. My participitation in this study primary involved performance of lumbar medial branch nerve radiofrequency ablation procedures.
