A. Personal Statement
As an anesthesiologist specializing in chronic pain management I have extensive clinical experience and expertise in spine interventions including facet injections, medial branch blocks and radiofrequency ablation (RFA). I have performed more than 500 cases of lumbar RFA (LRFA) at least.
I have prior experience in the conduct of retrospective analysis of repeated lumbar epidural steroid injections and their relation to subsequent opioid use and lumbar spine surgery.
My tendency to pay close attention to detail in attempt to minimize risks and maximize effectiveness has helped my clinical practice safer and perhaps more effective, and led to publication of an article proposing potentially safer practice of cervical epidural steroid injection. I have been working with Dr Pradeep Suri together clinically for 5 years in VA Puget Sound Health Care System (VAPSHCS), and shared many patients. He also helped me publish the aforementioned paper by reviewing and providing suggestions and constructive critiques.
Along with Dr Suri, I will be in charge of making sure the LRFA procedures are of high quality, consistent with what we describe in the research plan and the protocol. I will participate in monthly meetings to review study progress and recruitment, and more frequent meetings during the start-up and early recruitment phases.
In addition to my efforts with data collection, I will contribute to interpretation of results, the manuscripts related to this, and our future efforts to do the definitive large trial informed by this data.
The LRFA procedures described here fit very well with the existing practices for LRFA at our facility, where we have high standards for doing these procedures
As a practicing pain specialist with 30 year clinical experience, I will contribute my expertise to every aspect of study design and conduct of the research led by Dr Suri. My clinical background will be useful to the research team to make the study even more clinically relevant and significant regardless of the results.
