A. Personal Statement
My medical education, fellowship training and research interests make me well-suited to serve as co-investigator on this topic. During my last year of residency training, I was awarded by the American Society of Regional Anesthesia and Pain Medicine a funded fellowship to study the effect of dopamine on a rat model a Complex Regional Pain Syndrome created by loose ligation of the sciatic nerve. After my residency, I pursued a fellowship in acute and chronic pain management mostly treating patients with acute, subacute pain, and chronic neuropathic pain.
I am currently the section chief for acute pain management in the Anesthesiology department at the Feinberg School of medicine and the medical director for pain management at Northwestern Memorial Hospital. Over the last 20 years, I practice interventional pain management with a focus on spinal pain and minimally invasive spinal interventions, as well as perioperative and inpatient pain management. My clinical research focuses on the study of pain mechanism and the development of new diagnostic and therapeutic modalities for clinical pain conditions, including perioperative acute pain management. I am currently engaged in multiple clinical research studies in chronic pain and surgical pain treatment, including novel analgesia clinical trials and imaging as well as in clinical trials on optimal perioperative pain management in patients with substance use disorders. I am enthusiastic in applying my expertise in pain management, and clinical research in assisting in this study.

C. Contribution to Science
1. As a pain management fellow, I was interested in reversing the effect of a Complex Regional Pain Syndrome in an animal model and patients. In a rat model, I induced a neuropathic pain syndrome by performing a loose ligation around the sciatic nerve, and I was able to reverse the effect by injecting intraperitoneal then intrathecal dopamine. After my fellowship, I continued to pursue acute and chronic pain management research. My interest focused on preventing a transition from acute to chronic pain after surgical or mechanical insult. I have studied the effect of minimizing acute pain in the immediate postop period by providing continuous analgesia in the epidural space and along the peripheral nerves.

A prospective, randomized, double-blind, placebo-controlled, multicenter study of the SPRINT peripheral nerve stimulation for post-traumatic phantom limb pain
Goal: A multicenter, randomized, double-blinded, placebo-controlled, partial-crossover study conducted to collect data on the safety and effectiveness of percutaneous PNS for chronic neuropathic pain in amputees. The primary hypothesis was that percutaneous PNS provides clinically significant pain relief in a statistically significantly greater proportion of subjects than the placebo control group.

"REGAIN Trial" 2016-2021
A Randomized Controlled Trial of Regional Versus General Anesthesia for Promoting Independence after Hip Fracture
Goal: Evaluate outcomes of common anaesthesia techniques for hip fracture surgery, an event that occurs over 1.6 million times each year worldwide, through an international, multicentre, randomised trial.
