Interoperable Measures Facilitate Person-Centered Planning and Transitions Between Post-Acute Care and Home and Community-Based Services

This session will highlight a new assessment, Functional Assessment of Standardized Items (FASI), and two FASI-based performance measures for home and community-based service (HCBS) waiver programs. The functional assessment examines activities of daily living, instrumental activities of daily living, mobility, assistive device use, living arrangements, caregiver availability, and assistance. Importantly, this symposium will consider how the use of standardized items facilitates communication across settings. Researchers will understand the impact of interoperable data elements for examining healthcare quality across the post-acute care continuum. Our symposium will facilitate awareness of FASI among rehabilitation practitioners who work in post-acute care and HCBS. Practitioners and researchers will also learn about the value of interoperability and how they can support quality metrics by identifying personal priorities and mapping functional needs to service provision(s).
