Implementing standards of care in practice is one strategy to address both unwarranted variability in care and the gap between what we know from research and what we do in practice. We implemented a company-wide, multi-component initiative to reduce unwarranted variability in service delivery by standardizing specific clinical behaviors based on external evidence as a strategy for maximizing value. Using performance improvement and knowledge translation strategies, we established a multi-year plan including standardized outcome measures, data analytics with resulting clinical decision-making support, and targeted interventions. During this session, we will describe the process we used to scale these activities across more than 150 skilled nursing facilities, provide concrete examples of knowledge tools and measurement strategies, and discuss facilitators and barriers. We will also describe the contribution of a unique role, Clinical Knowledge Broker, and the outcomes of their work.

Learning Objectives:

Describe solutions to common barriers to knowledge translation.
Design a means of measuring fidelity of clinician behaviors with targeted behaviors that is feasible in multiple locations.
Develop a plan for implementation of a standard of care with their work group that includes 3-5 knowledge translation interventions.
