In the past 20 years, there has been a national trend revealing that there are 50% fewer admissions to acute IRF of polytrauma patients. With the evolving changes in the health care system, Atrium Health encountered a rise in unfunded patients following complex injuries that become “stuck” in the acute setting for months without the resources needed for a successful discharge. Acute care therapy lacks the resources and team structure present in IRFs and many patients lack resources to qualify for the IRF. Atrium Health’s HOPE for Home program is a model of rehabilitation that breaks down silos within organizations. This program provides the therapies of a IRF for acute care patients with disposition issues that prevent them from qualifying for IPR. This presentation will detail how resource allocation, interdisciplinary teamwork, and a new model of rehabilitation has resulted in cost savings for the hospital, reduced LOS and successful discharges.
