This presentation will discuss patient centered care and the use of patient goals as a driver of outcomes in brain injury rehabilitation. Goal attainment scaling (GAS) will be described as a key tool utilized to implement a patient centered plan of care, set expectations for success, and measure goal attainment. Methods of hardwiring programmatic implementation, common barriers and solutions to implementation, and measurement of programmatic outcomes will be presented for 3 brain injury rehabilitation programs: an inpatient rehabilitation program serving patients with moderate to severe brain injury, an intensive outpatient day program serving military service members and veterans with a history of mild brain injury and co-occurring psychological health conditions, and an outpatient clinic serving adolescents and adults with recent history of mild brain injury. Lessons learned and future directions will also be discussed.
