The goal of precision rehabilitation is to provide the right care for the right person at the right time. This model of care requires and facilitates a learning health system, where data are collected from patients, analyzed and interpreted, and ultimately fed back into the health system to improve patient-specific care for patients in the future. In this session, we will discuss strategies for development and implementation of a precision rehabilitation framework that combines clinical data from electronic health records and prospective, remote measurement of whole-person functional status (i.e., motor, cognitive, and psychosocial status) using wearables and web applications.

Learning Objectives:

Upon completion, participants will be able to identify strategies that can be used to provide precision (i.e., patient-specific) rehabilitation.
Upon completion, participants will be able to identify the important components of a precision rehabilitation team.
Upon completion, participants will identify how to combine clinical data and data collected remotely using readily available devices (e.g., Fitbits) to gain important insight into the functional status of each individual patient, which is critical to precision rehabilitaton.
