This symposium will provide an overview of the conceptual models that underlie programs to improve transitions from the hospital to post-acute care. It will include discussion of the components of these models and how their implementation impacts patient outcomes. Participants will have the opportunity to engage in discussion to identify strategies and future opportunities to improve engagement of rehabilitation providers in care transition model efforts at their home institutions.

Learning Objectives:

Describe components of care transition conceptual frameworks.
Identify outcomes associated with hospital-based interventions and post-acute care transition interventions.
Discuss how rehabilitation providers can engage in and advance the implementation of care transition models across the continuum of care to improve important outcomes.
