This symposium will discuss the development of Care Partner Carryover Day (CPCD), an interdisciplinary program for training care partners of patients with acquired brain injuries (ABI). CPCD was developed to provide a care partner-centered approach to increase timeliness of thorough education and skills training. It aims to improve care partners’ preparedness in caring for patients with ABI prior to discharge home from an adult inpatient neurorehabilitation unit. The program includes early identification of care partners, use of the Preparedness for Caregiving Scale as a tool to assess confidence and guide tailored educational interventions, and concludes with “Care Partner Carryover Day.” Through an interdisciplinary approach, care partners build the necessary skill set to improve readiness and decrease burden of care through the care receivers recovery. Utilizing our unique scopes of practice, clinicians can address the needs of care partners and care receivers via a formalized, client-centered, holistic care partner training program.

Learning Objectives:

Identify gaps in the literature and opportunities for change in current care partner training practices
Define the Care Dyad and the interdisciplinary team role in facilitating the care partnership between care receiver and care partner
Understand the role of a formalized care partner training program in improving care partners' preparedness in providing care for the care receiver upon discharge home
