Chronic brain injury is associated with specific neurologic, medical and psychological conditions that may decrease function, relationship quality, employment, quality of life, and life expectancy. Brain injury can evolve into a chronic condition that requires proactive management and surveillance to optimize outcomes for those living with the effects of Traumatic brain injury (TBI). Thus, people with chronic brain injury have a lifelong need for medical and community-based support to identify, treat, and prevent primary and secondary consequences. Unfortunately, this is not currently the standard of care experienced after brain injury. The majority of medical care for brain injury is rendered in the first weeks or months after injury, with care, support, and other resources dwindling over time. The 2013 Galveston Brain Injury developed a clear vision for the management of brain injury as a chronic condition: “Brain injury will be identified and proactively managed as a life-long chronic disease that will result in improvements in longevity, level of disability, expense to society, and quality of life. This will be achieved through modification of our medical and psychosocial health care delivery systems, health care education curricula, research agenda, and legislative/health care policy.”   To address this vision, a self-directed chronic care model is being developed: The BeHEALTHY Model. This presentation will provide researchers and clinicians with information on the evidence base for chronic care and self-management in populations with relevance for TBI, as well as on the clinical guidelines for management of TBI-related difficulties. The presentation will also describe a preliminary innovative and person-centered chronic disease model for people with TBI with the opportunity for input to refine the model.
