Research Objectives: To apply a measure of neuromechanical function - the local dynamic gait stability index (Dingwell & Cusumano, 2000) - concurrent with prospective 6min-walk test (6mwt) assessment of intervention for chemotherapy-induced neuropathy (CIN).

Design: Case study; first-in-human application for (a) neuromechanics assessment within the 6mwt and (b) change with intervention for CIN

Setting: Oncology outpatient (recruitment); community (intervention)

Participants: One breast cancer (BC) survivor with CIN and balance deficits defined per Worthen-Chaudhari (2018).

Interventions: AdapTango dance (Tango) (Hackney, 2010; Worthen-Chaudhari, 2019) performed 2x per week for 8 weeks. Approximately 15 min partnered movement-to-music dose per session.

Main Outcome Measures: Primary: Local dynamic gait stability index (LSI) (Sloot et al, 2011), a non-linear analysis measure of neuromotor stability during steady-state gait (Dingwell & Cusumano, 2000) & fall risk (Lockhart & Liu, 2009). LSI calculates maximum, short-term, finite-time Lyapunov exponents, a measure of a person’s resistance to their own between-stride movement variability. We measured LSI continuously using portable equipment (two inertial measurement units (IMUs; Delsys Trigno) at the torso and foot) while survivors performed the 6mwt (Galiano-Castillo, 2016) overground pre/post intervention.

Results: LSI improved with intervention (p < 0.0001, t-test; within-subject Cohen’s d effect size = 1.52; mean(SD) pre = 0.80(0.18) and post = 0.51(0.20)).

Conclusions: Tango dance improved dynamic gait stability for 1 BC survivor with neuropathy. LSI (Sloot, 2011) is feasible to collect during clinical neurorehabilitation gait assessment (6mwt); clinical utility requires further study.
