Goal Priorities for Coaching in Community-Dwelling Individuals with Stroke and their Caregivers

Research Objectives: To identify goal priorities among individuals with stroke and their caregivers participating in a coaching intervention in order to inform the design of community-based coaching programs.

Design: Descriptive study.

Setting: General community.

Participants: Caregivers (n = 9) and individuals with stroke (n = 3) participating in Coaching-in-Context intervention.

Interventions: Not applicable.

Main Outcome Measures: The primary outcome was identification and classification of coaching goals most frequently reported by participants on the Canadian Occupational Performance Measure (COPM).

Results: During initial coaching sessions with a certified coach, participants generated 88 goals which can be classified into 4 main types: rehabilitation-related (92% of participants), self-care (83%), household or job responsibilities (83%), and relationships (42%). Among rehabilitation-related goals, 87% of caregivers identified goals of improving caregiving skills or emotionally supporting the receivers of their care, and 100% of individuals with stroke identified goals of improving stroke-related deficits, such as mobility or speech. Among self-care goals, 67% of participants identified goals of personal care and 67% identified goals of leisure/recreation. Personal care often involved stress management or healthy diets, and leisure/recreation involved isolated relaxation or physical outdoor activities. 67% of participants also identified household management goals, especially balancing multiple family roles, cleaning, and finances.

Conclusions: This study provides insight into the main problem areas that individuals with stroke and their caregivers choose to address when given the opportunity to participate in coaching. Tailoring community-based coaching programs to the goal priorities of this population is important for enhancing post-discharge support.
