Comparison of detrusor-sphincter dyssynergia in neurogenic and non-neurogenic bladder

Research Objectives: to determine the difference of dyssynergic sphincter activity at urodynamic study in neurogenic and non-neurogenic bladder

Design: retrospective review of medical records.

Setting: bladder problem patients with detrusor-sphincter dyssynergia on urodynamic study
university hospital setting.

Participants: Observational study.
total 162 patients who had urodynamic study for bladder symptoms

Interventions: review of medical records with urodynamic study

Main Outcome Measures: urodynamic parameters: detrusor sphincter dyssynergia, bladder volume, detrusor pressure at maxiamum flow, detrusor leak point pressure,

Results: A total of 162 patients were reviewed and 56 patients were excluded. There were 31 and 75 patients in the NB and non-NB groups, respectively. Detrusor sphicnter dyssynergia (DSD) was observed in the non-NB group with prevalence of 21.3% which was significantly lower than that of 58.1% observed in the NB group (p < 0.001). DSD in non-NB showed 100% of type 1, whereas DSD in NB showed type 1 (77.8%), type 2 (5.56%), and type 3 (16.6%). Cystometric capacity and detrusor pressure at maximum flow (PdetQmax) were significantly higher in the NB group than in the non-NB group. Among all patients, detrusor leak point pressure and PdetQmax were significantly higher in patients with dyssynergic sphincter activity than in patients without dyssynergic sphincter activity

Conclusions: The prevalence of DSD in non-NB patients was relatively common. The dyssynergic sphincter pattern in NB had different presentations, in comparison with dyssynergic sphincter patterns of the non-NB cohort.
