The session will focus on QI approaches to address troubling disparities in health outcomes. Moderated by physicians and improvement advisors, the session will focus on use of QI methodology to assess for quality care gaps related to factors associated with health disparities (e.g. race, ethnicity, language other than English, insurance status, and other social determinants of health), how to identify health outcomes disparities, and interventions to help mitigate them.
Content will also draw from experience of centers participating in a learning health network to improve outcomes of care in pediatric rheumatic disease. It will highlight a registry-based population management approach, efforts for inclusion in data analytics, and QI interventions to close gaps in care, including reliability in health condition screening, provision of self-management support, overcoming barriers to adherence, and a treat-to-target intervention for tighter disease control.
Additionally, each session will touch on topics, including effecting changes in delivery system design, decision support, and clinical information systems.
