A complex relationship exists between rheumatic diseases and cancer, particularly for patients with hematologic malignancies. Large granular lymphocyte (LGL) leukemia is at the intersection of chronic inflammation, lymphoproliferative disease, and autoimmunity. Pathogenesis and management of LGL in patients with rheumatic diseases will be discussed.
