Sleep disturbance is a public health problem impacting individuals across the lifespan. Approximately one third of adults (Liu et al., 2014) and 75% of adolescents (Owens, 2014) get insufficient sleep. Sleep and circadian problems frequently co-occur with, predict, and predate mental health problems (e.g., Hertenstein et al., 2019), and often do not resolve following treatment focused on the mental health problems (e.g., Nierenberg et al., 2010). Many patients who enter treatment for a mental health problem have sleep problems (Seow et al., 2018) and treating sleep alone can improve other mental health symptoms (e.g., Ulmer et al., 2011). Behavioral sleep medicine approaches, such as cognitive behavior therapy for insomnia (CBTI), have been established as the front-line treatments for many sleep problems. However, given the prevalence of sleep and circadian problems and the high rates of comorbidity with mental health problems, it is particularly concerning that there is a scarcity of behavioral sleep medicine providers (Thomas et al., 2016). This symposium will highlight: (1) the importance of delivering sleep treatments and increasing trained sleep providers in routine care settings, (2) novel methods of disseminating and evaluating evidence-based sleep care, and (3) innovative strategies to train clinicians. Presentations will highlight multiple evidence-based sleep and circadian interventions, address multiple populations and settings, and span clinician training to patient outcomes.
The first speaker will present data displaying the lack of clinician training in behavioral sleep medicine. The second speaker will describe results from a free, online program to increase the number of trained CBTI providers. The third speaker will share findings highlighting how modules from the Transdiagnostic Sleep and Circadian Intervention (TranS-C), an evidence-based treatment for sleep and circadian dysfunction and comorbid mental illness, are not always delivered as intended, and the association between TranS-C delivery and treatment outcome in a publicly-funded mental health setting. The fourth speaker will present data evaluating the impact of the quantity of TranS-C modules delivered on patient outcomes with an eye towards strengthening guidelines for providers and streamlining training efforts. The fifth speaker will highlight findings from a clinical trial of TranS-C delivered via app in a youth primary care setting. Finally, the discussant, who is an expert in implementation science, will discuss themes, critique the findings, and identify future directions for the implementation of evidence-based behavioral sleep medicine practices.
