Recent studies have shown that only 71% of clinicians who regularly practice Cognitive Behavioral Therapy (CBT) view themselves as highly competent in their field and/or report higher levels of self-efficacy (Creed, et al., 2016). In reality, all psychologists should feel competent in their abilities as clinicians. Psychologists can be trained to conduct psychotherapy through the use of role-playing activities, yet there is little existing research on other simulation-based methods of cognitive-behavioral training (SBMoCBT) (Sheen et al., 2021). We defined SBMoCBTs as experiential-behavioral methods of instructing clinicians on CBT techniques that involve interaction with a virtual client via simulated patients, virtual roleplays, and/or objective structured clinical examinations (OSCEs) with feedback about performance. The purpose of this study was to run a meta-analysis to determine whether simulation-based models of training would improve clinicians’ perceived abilities to implement CBT with their clients and its relation to clinical outcomes. Building off the work of Ericsson, who demonstrated competency using simulated-based learning, we screened for articles that had at least 2.5 hours of simulated-patient-practice with a supervisor feedback session of at least 45 minutes to be included in data analysis. This research aims to answer the following: (a) what are the psychosocial factors contributing to lower levels of perceived confidence?, (b) what are the measurements for competency in simulation-based training?, (c) are simulation-based methods of training a good method of improving competency and client outcomes? We hypothesized that simulation-based training methods would lead to increased levels of perceived and observed competency and self-efficacy compared to traditional CBT training groups. Through ongoing research, preliminary results show that those SBMoCBT protocols that meet our operational definition have shown significant differences in comparison with CBT and treatment as usual on client/clinician improvement and outcomes. There is considerable variability across studies in the methodology and specific SBMoCBT approaches utilized and as a result, we offer research and clinical training suggestions.
