PC3077: Improving Resident Experience During the Pediatric Intensive Care Unit Rotation

Purpose/Objectives: It was recognized through rotation evaluations that residents were dissatisfied with their Pediatric Intensive Care Unit (PICU) experience. It is essential to consistently review residents’ satisfaction and intervene in a timely manner as it has been identified that resident work dissatisfaction may impact resident well-being. Through confidential discussion with residents and PICU rotation evaluations, key drivers were identified (Figure 1). A multidisciplinary PICU improvement team was formed, a key driver diagram was created, and interventions were developed. The aim was to have at least 50% of residents confirm the implemented processes were beneficial to their PICU experience by the completion of their rotation.

Design/Methods: The key driver diagram focused on 5 areas: expectation of knowledge, structure of rounds, resolution of conflicts, patient load, and nursing experience. Interventions began in April 2020. The improvement team met quarterly and were in constant communication discussing the status of implementation. The improvement team met quarterly, but consistently discussed the status of implementation. Nine months after implementation, anonymous satisfaction surveys were completed by residents.

Results: The survey was filled out by 12 senior residents. Fifty percent or more of the residents felt the interventions assessed in the survey were beneficial. All residents who were exposed to PICU before their formal rotation, who participated in bedside teaching and who completed the online modules found it beneficial. Overall resident satisfaction for each PICU attending and the PICU rotation as a whole is shown in Figure 2.

Conclusion/Discussion: This study demonstrates that timely and successful creation of an improvement team in response to resident dissatisfaction can be beneficial to rotation experience and residents’ well-being.
