Post Visit: For Patients: Patient-specific education for printing and available in an individual portal page with virtual a baby book for each child; reminders to complete monitoring online for chronic conditions; For Clinicians: Clinical references; e-Chapters; MOC-4 credits; clinical case consultation via webcast meetings; registries and reports supporting PCMH and CMS requirements.
